II RESPONSIBILITIES AND AUTHORITIES:
Admissions and discharges:
Incumbent is responsible for coordinating all admissions to and discharges from the facility.
Incumbent maintains an awareness of the current census, potential admissions, and potential discharges.
All admission referrals are reviewed by the incumbent to determine if the individual meets admission criteria for the services offered by The Home.
Incumbent coordinates admissions working with and supporting the parents/guardians, explaining how The Home is organized, the different functions of each department, policies and procedures in place to provide a safe environment for the residents served.
Incumbent plans for and processes discharges working with the parents/legal guardians and community resources; identifies appropriate placement options and working with other departments, completes a smooth transition from The Home.
Incumbent participates in marketing The Home’s services as needed; also chairs the Outreach Committee which meets regularly to review referrals and to identify and cultivate potential new referral sources.
Advocacy and Support:
The Social Worker maintains a clearinghouse for information on the child and family.
Incumbent compiles family/social histories quarterly and annually.
Participates in individual service plan meetings and assists with program development.
Performs case management, identifying community resources that are needed at certain times during the resident’s stay.
Maintains and promotes professional relationships inside the Home and in the community.
Incumbent also supports the family/guardians throughout the resident’s stay by responding to concerns raised, encouraging and assisting involvement with their child, clarifying policies and procedures, maintaining a list of those individuals who may not visit a particular resident, assisting with resolving conflicts, or consoling parents of a deceased child.
Compliance with Regulations and Client Relations:
The Social Worker is familiar with Medicaid and DBHDS (ICF) regulations, human rights bylaws and regulations, and the behavior management manual and serves as the Human Rights Representative for the facility.
Represents The Home at quarterly Local Human Rights Committee meetings and reports to that committee. Also reports any complaints, abuse, or neglect allegations in CHRIS (Comprehensive Human Rights Information System).
Conducts a thorough investigation and submits a plan of correction for each event.
Reports to The Home’s licensing agency, Department of Behavioral Health and Disability Services (DBHDS) on any abuse, neglect, deaths, or serious injuries and responds to requests to licensure regarding the results of the investigation.
The social worker maintains records and participates in annual and quarterly meetings for each child.
Serves on several committees including quality improvement, behavior management and team meetings on individual children.
Monitors the observance of human rights, client rights, confidentiality and privacy.
The Social Work department is responsible for investigating any abuse and neglect allegations, deaths, and any other serious incidents that take place in the facility and to take appropriate action.
Incumbent is expected to keep abreast of current knowledge and trends as it relates to the provision of social services provided to the residents at The Home, attending conferences and seminars as needed.
