    JOB SUMMARY:    Provides specialized nursing care and treatments for critically fragile individuals with special medical and physical needs at St. Mary’s Home. Incumbents may be required to work a variety of shifts and fill the charge nurse role when scheduled. All services will be provided under a person centered model of care and in support of each individual’s support plan (ISP).  We offer a competitive rate of pay and we recently implemented a nursing sign-on bonus of $4,000.00!!





    1. Conduct daily physical assessments of assigned individuals.
    2. Administer medications, treatments and tube feedings as prescribed by the physician.
    3. Receive, transcribe and implement physician’s orders using the electronic health record.
    4. Manage and provide care to tracheostomy-dependent and ventilator-dependent individuals after completing the required training.
    5. Intervene appropriately in the event of emergencies with individuals or staff.
    6. Document activities related to nursing and medical care in a timely manner and consistent with established formats.
    7. Safeguard Individual Rights.
    8. Assist in developing and updating the program plans of individuals when requested.
    9. Be knowledgeable about, and participate in the provision of Active Treatment programs for individuals.
    10. Assist with the maintenance of a safe and sanitary environment in compliance with OSHA regulations.
    11. Participate in staff meetings and staff development programs as directed.
    12. Communicate with families and staff.
    13. Accompany individuals on appointments or field trips when requested.
    14. Direct/supervise the delivery of nursing care in the absence of the Director of Nursing or an RN.
    15. Act as Charge Nurse as assigned.
    16. Be prepared to precept new nurses, if asked.
    17. Direct and assist the activities of the Direct Support Professionals (DSPs) in the absence of the Program and Assistant Program Managers.
    18. Report insubordinate DSP behavior or any other flagrant disregard of DSP job assignments, duties or care procedures for individuals to the Program or Assistant Program Managers.
    19. Follow Event reporting procedures for instances of neglect and abuse.
    20. Attends all in-services posted as mandatory and completes additional fifteen hours of related training every year.

            10. Be prepared to work an extended shift if nursing coverage is required.



          1. Must be a graduate of an accredited school of practical nursing.
          2. Must have a current Practical Nurse license issued by the Commonwealth of Virginia or proof of application for such.
          3. Prefer one year of experience, preferably in pediatrics or medical/surgical areas.
          4. Must have the ability to make frequent nursing judgements and/or decisions.
          5. Knowledge, skill and experience in specialized nursing techniques, including the management and care of tracheostomy-dependent and ventilator-dependent individuals preferred.
          6. Receive references that reflect a good attitude, dependability, good judgement, and the ability to function independently.
          7. Have a current CPR certification.


          1. Must be able to sit, stand and walk for variable periods of time.
          2. Must be able to lift up to thirty-five (35) pounds independently, and with or without required equipment (dependent on the weight of the individual), be able to safely lift individuals to reposition them, place them in chairs etc.
